Medical

ANTHEM BLUE CLASSIC PPO
You want this if:
You’d rather pay a bit more each paycheck to save on medical costs later. It has the lowest deductible and out-of-pocket maximum, and it’s the only plan that includes copays for doctor and specialist visits. A great fit if you have regular medical needs or expect higher costs, like a pregnancy or upcoming procedure.
- In-Network Deductible (individual/family) $1,000 / $3,000
- In-Network Out of Pocket Max $4,000 / $12,000
- Copays: $25 PCP and $50 Specialist
- Coinsurance 20%
- Emergency Room Copay $250
- Rx Copay $15 / $40 / $70 (administered by CVS Caremark)

Dental
East West offers two options for dental coverage: a base plan and a buy-up plan. The major differences are in the annual deductible, annual maximum per person, and Orthodontia coverage.
ANTHEM DENTAL BASE PLAN
You want this if:
You anticipate a basic level of dental care, such as check-ups, cleanings, and some common treatments. It’s a good option to cover the routine care while also offering some payment towards unexpected dental costs.
- Deductible (individual/family) $100 / $300
- Per person annual benefit maximum $1500
- 100% coverage on diagnostic and preventative
- 80% coverage on basic services after deductible
- 50% coverage on major services after the deductible
- NO Orthodontia coverage

Vision
East West offers two vision plans through Anthem Vision. Both the Base and Buy-up Plans provide in-network and out-of-network coverage. You pay less out-of-pocket when you see an in-network provider.
ANTHEM VISION BASE PLAN
You want this if:
You want essential eye care, such as routine exams and discounts on glasses or contacts. It’s a simple option for those looking to support maintaining healthy vision and managing everyday eye care costs.
- $10 in-network exam copay
- $20 in-network material copay
- $130 in-network allowance on frames and elective contacts
- Lenses 100% after copay
- 12-month frequency period for exam, lenses, and contacts
- 24-month frequency period for frames

TRANSCARENT
You want this if:
You have a planned non-emergent surgery and want to pay $0 out of pocket*. Simply connect with a care team member at Transcarent, and they will walk you through the steps from discussing care options, selecting a surgeon at one of their top-quality centers of excellence, preparatory steps for surgery, including travel arrangements, through recovery.

*IRS requires a minimum deductible is met when on an HSA-qualified plan. This means if you are on the PPO or HDHP plans, your surgery is covered 100%. If you are on the HSA Medical Plan, you must first meet the IRS minimum deductible and then it is covered 100%.
