Supplemental coverages offer additional security and peace of mind. They can be elected a la carte or paired with a medical plan to help cover unexpected expenses. Voluntary Accident Coverage
Accident Insurance pays you benefits for specific injuries and events resulting from a covered accident. Benefits will be paid directly to you to use for any purpose, such as paying out-of-pocket medical expenses, copays, deductibles, groceries, gas, utilities, and more – it’s up to you.
Voya Accident
You want this if:
You are enrolled in the Anthem Blue Classic PPO plan or waiving Medical coverage. You have the option to enroll yourself as well as your spouse and children. It is not health insurance and does not satisfy the requirement of minimum essential coverage under the ACA; however, in the event of an accident, it can help cover some costs. Critical Illness
Critical Illness Insurance pays a lump-sum benefit if you are diagnosed with a covered illness or condition. Critical Illness Insurance is a limited benefit policy, it is not health insurance and does not satisfy the requirement of minimum essential coverage under the ACA.
You want this if:
Use your paid benefit of $5,000, $10,000, $20,000, or $30,000 for any purpose, such as paying out-of-pocket medical expenses, copays, deductibles, groceries, gas, utilities, and more – it’s up to you. Hospital Indemnity
Hospital Indemnity Insurance pays a fixed daily benefit if you have a covered stay in a hospital, critical care unit or rehabilitation facility. Hospital Indemnity Insurance is a limited benefit policy, it is not health insurance and does not satisfy the requirement of minimum essential coverage under the ACA.
You want this if:
Use your paid benefit for any purpose, such as paying out-of-pocket medical expenses, copays, deductibles, groceries, gas, utilities, and more – it’s up to you. Visit the Voya Benefits Resource Center for more information.
How it works:
First day of confinement (Admission Benefit), you would receive $600 for a hospital or rehabilitation facility admission or $1200 for a Critical Care Unit admission. Starting day two, you would receive $100 per day of confinement in a hospital or rehabilitation facility, or $200 per day of confinement in a Critical Care Unit. Payment is eligible for up to 30 days of confinement.

Voluntary Life
East West Family of Companies provides basic Group Term Life Insurance to you at no cost at 1.5x your annual salary up to $200,000 or $70,000 if you are a restaurant-tipped employee or a commissioned employee. The company also covers $5,000 in coverage for your spouse and $2,500 for your child(ren). This is provided to all benefit-eligible employees.
Why should I consider more?
You want this if:
In addition to the basic coverage being provided to you at no cost to you, you have the option to elect additional coverage called Supplemental Life Insurance. Not sure how much you need? Try the Life Insurance Calculator at go.voya.com/lifecalc.
Amounts you can elect:
For you
- $10,000 up to a maximum of $500,000 in $10,000 increments, not to exceed 5 times basic annual earnings.
- Guaranteed Issue Amount of $150,000 when first eligible to make an election.
- This is the amount you can elect up to without needing to provide Evidence of Insurability.
- Amounts greater than the Guaranteed Issue will be pending a review of a medical questionnaire called Evidence of Insurability (EOI). Voya will send an email with a secure link to complete this. Your health information is private and protected. No one from East West will see your answers.
For your Spouse
- $5,000 to a maximum of $250,000 in $5,000 increments, not to exceed 50% of the employee’s Supplemental Life insurance amount.
- Guaranteed Issue Amount of $25,000 when first eligible to make an election.
- You must have an election for yourself in order to elect spousal coverage.
- If your Spouse is eligible for coverage as an employee of East West, they are not eligible for additional coverage as a spouse.
For your Child(ren)
- $2,500 up to a maximum of $10,000 in $2,500 increments.
- Guaranteed Issue Amount of $10,000 when first eligible to make an election.
- You must have an election for yourself in order to elect coverage for your child(ren).
- Qualified Child dependents under the age of 26.
- If your child is eligible for coverage as an employee of East West, they are not eligible for additional coverage as a child.
Guaranteed-Issue Amount and Evidence of Insurability
The guaranteed-issue limit is the amount that’s available during this enrollment period without providing evidence of insurability (EOI). To get coverage beyond this limit or add/increase coverage after this enrollment period, you’ll need to complete the EOI form for all applicable family members. This form includes questions about current and past health conditions. The insurer may request additional information before approving or denying coverage. When evidence of insurability is required, the insurance company will need to approve it before coverage becomes effective.

Accidental Death & Dismemberment

You want this if:
In addition to the basic coverage being provided to you at no cost to you, you have the option to elect additional coverage called Supplemental Accidental Death & Dismemberment Insurance, which provides the insured person or their beneficiary a payment separate from the life insurance benefit if the insured person dies or is severely injured in a covered accident. There is no Guaranteed Issue Limit with AD&D, as the cause of death or severe injury must be due to an accident and not health-related.

For you
- $10,000 up to a maximum of $500,000 in $10,000 increments.
For your Spouse
- $5,000 to a maximum of $250,000 in $5,000 increments, not to exceed 50% of the employee’s Supplemental Life insurance amount.
For your Child(ren)
- $2,500 up to a maximum of $10,000 in $2,500 increments.
- You must have an election for yourself in order to elect coverage for your child(ren).
